Booking Form with contact info

1. Please read the Pay for a Ticket information page and see the fee transfer information there.
2. Please complete this Booking Form for each individual.

• Please confirm if you are you camping and so require a tent pitch;
• or if you are sharing a tent with someone else who has booked;
• or are you arranging off-site accommodation (where will that be?).
• Cost of the Saturday meal together is part of the booking fee:
  say if you want a vegetarian or vegan option for the dinner.
• Give dates and times you expect to arrive at and leave the event.
• Can you give your own phone number and emergency contact details in case we need to contact someone else on your behalf?
• Any particular requirements / other info you want us to be aware of?
• Confirm you have transferred either £39, or £20, for the booking ticket.
